ABSTRACT

The implantable collamer lens(ICL)is a widely popular option for the correction of refractive errors. ICL implantation brings a whole new dimension to the vision, from the anterior chamber phakic ICL to the posterior and central hole ICL. Even though there are fewer reported complications, ICL size selection remains challenging due to the differences in device measurements and ICL sizing formulas. With the widening comprehensiveness of ICL implantation and the ongoing development of ophthalmic devices and technologies, particularly the advent of artificial intelligence, more and more indicators such as sulcus-to-sulcus(STS), crystalline lens rise(CLR), angle-to-angle(ATA), the iris pigment end to the iris pigment end(PTP), anterior chamber width(ACW), and anterior chamber angle(ACA)are providing references in the selection of ICL size, this article provides a review of ICL size selection.

ABSTRACT

OBJECTIVE@#To explore the expression levels and clinical significance of helper T cell 1/helper T cell 2 (Th1/Th2) cytokine and interleukin-6 (IL-6) in patients with acute leukemia (AL) complicated by infection.@*METHODS@#68 patients with AL complicated by infection admitted to Wuhan Fifth Hospital from May 2017 to January 2020 were enrolled as study group, 50 AL patients without infection were enrolled as AL group, and 30 healthy volunteers checked in physical examination center were enrolled as healthy control group. The levels of serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and interleukin-10 (IL-10), and peripheral blood Th1/Th2 cells subsets were measured and compared among the three groups. The serum IL-6, IL-10, TNF-α and Th1/Th2 were compared between the patients with mild to moderate infection (n=52) and septic shock (n=16). The relationship between IL-6, IL-10, TNF-α, Th1/Th2 and AL infection was analyzed.@*RESULTS@#The levels of IL-6, IL-10 , TNF-α, and the proportion of Th2 of the patients in study group and AL group were significantly higher than those in healthy control group (P<0.001), while the proportion of Th1 and Th1/Th2 were significantly lower than those in healthy control group (P<0.001). The levels of IL-6, IL-10 and TNF-α, and the proportion of Th2 the patients in study group were significantly higher than those in AL group (P<0.001), while the proportion of Th1 and Th1/Th2 were significantly lower than those in AL group (P<0.001). The serum IL-6, IL-10 and TNF-α level of the patients in septic shock group were significantly higher than those in mild-to-moderate infection group (P<0.001), while Th1/Th2 was lower than those in mild-to-moderate infection group (P<0.001). The results of ROC curve analysis showed that the area under the ROC curve (AUC) values of IL-6, IL-10, TNF-α and Th1/Th2 alone for the diagnosis of septic shock were 0.779, 0.761, 0.724 and 0.718, which were lower than that their combination (0.910) (P<0.05).@*CONCLUSION@#The levels of serum IL-6, IL-10 and TNF-α are high in patients with AL complicated infection and septic shock, while Th1/Th2 cell subsets is low. The combined detection of serum IL-6, IL-10, TNF-α and Th1/Th2 is a good diagnostic value for predicting the occurrence of severe septic shock.

ABSTRACT

BACKGROUND:Keratitis,chemical injury,trauma,and other diseases often lead to irreparable damage to the corneal tissue,which can cause scars and finally vision loss.Corneal transplantation is the only way for treatment,but is of limit use by the deficiency of donors.Moreover,biosynthetic implants are expensive and complex.Silk fibroin and chitosan are common materials for tissue engineering and have been widely used as bio-scaffold materials.However,the technology applied in corneal repair is still in infant stage.OBJECTIVE:To systemically analyze the literature regarding the application of silk fibroin/chitosan scaffold materials in corneal repair,and to offer new directions and theoretical support for further studies on corneal repair.METHODS:A computer-based search of CNKI,WanFang and PubMed was performed for relevant articles with the keywords of "silk fibroin,chitosan,cornea,keratoprosthesis,scaffold".Then,we traced the development progress of silk fibroin/chitosan scaffolds in corneal repair and summarized its current applications and prospects.RESULTS AND CONCLUSION:In total,178 articles were preliminarily retrieved,and finally,52 eligible articles were reviewed.(1) The silk fibroin/chitosan scaffold for corneal repair has low cytotoxicity and good biocompatibility.(2) This kind of scaffold supplies proper pericellular environmental for corneal cells that is good for hydroelectrolyte exchange.(3) The presence of ABCG2 protein implies that the scaffold offers possibilities of limbal stem cell proliferation.(4) Increasing animal model tests have shown that the scaffold can barely cause an inflammatory reaction of corneal cells,which is highy important to remain the vitreousness of corneal tissues.(5) Because of the deficiency of corneal donors,searching for new artificial materials for corneal repair is the developing direction of corneal transplantation.Silk fibroin chitosan scaffold has huge potential in this area,and it deserves more researches for clinical use in the future.

ABSTRACT

·High altitude retinopathy (HAR) is the pathological changes of retina and optic caused by hypobaric hypoxia when the body is in high altitude environment, and HAR is the major cause of visual loss for the plainsmen who went to high attitude area. With the development of economy and tourism of plateau regions, the prevalence of HAR has recently been increasing for more and more people going to high altitude areas. Currently, it is reported that the pathogenesis of HAR involves a host of mechanisms, including the integrity damage of blood-retinal barrier (BRB),retinal neuron injury,inflammatory mediators and cytokines produced excessively, retinal blood vessels regulate dysfunction, and hemodynamics abnormality,and the synergistic action between them. In this review, we report the findings from current studies on the pathogenesis of HAR, contributing to further studies and diagnosis and treatment.

ABSTRACT

·AIM: To investigate the effects of astragalus injection on the metabolic and pathological changes of the retinal free radical metabolism and pathology in high-altitude hypoxic rats. ·METHODS: Sixty healthy SD rats without any eye disease were selected,and the random grouping method was divided into two groups: astragalus injection group (intervention group) and physiological saline group (control group),each group of 30 rats. In each group,6 were randomly selected for injection of astragalus injection (15mL/kg) and physiological saline (15mL/kg) in the pre-simulated chamber. The rats were sacrificed immediately and removed the eyeballs after the rats were taken out of the simulated module. We observed the changes of retinal morphology with the HE staining method, and determine the retinal SOD and MDA content with colorimetry. · RESULTS: HE staining showed no morphological changes in the two groups of retinas at 2h, and as the time of hypoxia was prolonged, the retinal edema gradually increased,but the intervention group was less edema than the control group. The activity of SOD in both groups decreased with the increase of anoxia time of high altitude, and the comparison of different time points in the group was statistically significant(P<0.05). The content of MDA in both groups increased gradually, and the comparison between different time points in the group was statistically significant (P<0.05). The SOD in the two groups of retinas was significantly different at same time point(P<0. 05), except for at 2h without statistical significance. The MDA showed the same situation as SOD. ·CONCLUSION: Astragalus injection can reduce the damage degree of retinopathy in rats under the high altitude hypoxia environment, that the mechanism may be related to free radicals, enhance the activity of SOD, reduce the MDA content of lipid peroxides,and enhance antioxidant capacity.

As the increasing population aging,the incidence of age-related macular degeneration is increasing,blinding rate also increasing,so it is very important for the early diagnosis and treatment of age-related macular degeneration.There are many methods to check the aging macular degeneration,such as fundus angiography,optical coherence tomography (OCT),visual field and multifocal electroretinogram (mfERG).In recent years,many emerging ophthalmic methods have emerged and are gradually applied to clinical diagnosis,including optic coherence tomography angiography (OCTA).The function of these methods has its unique advantages,but there are also limitations.This paper will review these existing methods.

ABSTRACT

Because of the exist of complex matrix, the confirming indicators of qualitative results for toxic substances in biological samples by chromatography-mass spectrometry are different from that in non-biological samples. Even in biological samples, the confirming indicators are different in various application areas. This paper reviews the similarities and differences of confirming indicators for the analyte in biological samples by chromatography-mass spectrometry in the field of forensic toxicological analysis and other application areas. These confirming indicators include retention time (RT), relative retention time (RRT), signal to noise (S/N), characteristic ions, relative abundance of characteristic ions, parent ion-daughter ion pair and abundance ratio of ion pair, etc.

ABSTRACT

AIM:To investigate the effects of 1.8mm coaxial micro incision phacoemulsification on corneal endothelial injury and postoperative visual acuity.METHODS: Totally 145 eyes in 120 patients underwent phacoemulsification from July 2013 to July 2015 were randomly divided into observation group 60 cases (73 eyes) and control group 60 cases (72 eyes).The observation group 60 cases were given 1.8mm coaxial micro incision cataract phacoemulsification operation,while the control group were given traditional 3.2mm coaxial micro incision cataract surgery.The uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),corneal thickness of incision area,incision width,incision length,macular retinal thickness,surgically induced astigmatism,corneal endothelial cell counts and complications of the two groups were compared.RESULTS: The UCVA and BCVA on 1wk after surgery of the observation group were significantly higher than the control group (t=3.604,7.109;P0.05).CONCLUSION: The 1.8mm micro incision phacoemulsification is helpful to improve the visual acuity of patients with cataract phacoemulsification,which may be related to the reduction of corneal cell injury,enhancement of corneal closure and decrease post-operation corneal original astigmatism.

ABSTRACT

OBJECTIVES@#To explore the change rules of behavioral characteristics, survival time and saturation of carboxyhemoglobin (HbCO) in different CO concentration to provide experimental basis for the cases of CO poisoning death in forensic practice.@*METHODS@#Total 160 SD rats were randomly divided into four groups. CO with the concentration of 1 250 mg/m³, 3 750 mg/m³, 6 250 mg/m³ were continuously and respectively replenished in a self-made toxicant exposure equipment until rats died from poisoning. In different CO concentration, the behavioral characteristics and survival time of poisoning rats were observed and recorded. The saturation of HbCO in heart blood was detected by spectrophotometric method. Organs such as brain, heart, lung and liver, and heart blood were obtained via autopsy and histopathological observation was performed.@*RESULTS@#The behavioral characteristics of CO poisoning rats were limp and slow response. There were a gradual decrease of survival time and an increase of HbCO saturation in rats with the increase of CO concentration. Three rats in CO concentration of 1 250 mg/m³ group showed lower saturations of HbCO than the lethal dose and this situation hasn't been found in other groups.@*CONCLUSIONS@#The animal model of CO poisoning established under different CO concentration has the advantages such as simplicity and good repeatability, which lays a foundation to the further study for CO and other inhaled toxic gas in the research of forensic sciences.

ABSTRACT

Abstract?AIM: To obtain the prevalence and risk factors of symptomatic dry eye disease ( SDED ) among college students in China.?METHODS:Population-based cross-sectional study. Students in Medical School of Lanzhou University were approached. A questionnaire was used to evaluate the prevalence of SDED and its risk factors. The diagnosis of SDED was based on reported symptoms and was established if the participants reported “often” or “all the time” once or more for 6-item questionnaire. Positive tests included a tear-film breakup time ( TBUT)≤10s and a fluorescein staining score ( FSS ) ≥1. Demographic information and possible factors that may contribute to SDED were analyzed in a step-wise multivariate logistic regression modelto assess risk factors of SDED.? RESULTS: There were 1139 participants ( 84. 37%response rate ) have completed the questionnaire, 475 males and 664 females aged 16-26y. The prevalence of SDED was 18. 70% [95% confidence interval ( CI)= 16. 59-20. 81]. A TBUT of ≤10s and a FSS≥1 were noted in 47. 67%(95% CI=44. 95-50. 57) and 13. 97%(95% CI=11. 95-15. 99) for all participants, respectively. The multivariate regression analysis revealed the following risk factors:daily reading time of≥4h(OR=1. 58,95% CI=1. 15-2. 18), daily computer use of≥4h ( OR= 1. 52, 95% CI= 1. 02-2. 25), and constant eyeglasses wearing (OR=1. 54,95%CI=1. 08-2. 13). The female gender, refractive surgery and contact lens ( CLs) wearing were not risk factors for SDED in this analysis.? CONCLUSION: The prevalence for SDED is high in Chinese college students. The risk factors include daily reading time of≥4h, daily computer use of≥4h and constant eyeglasses wearing.

ABSTRACT

Trachoma, a contagious keratoconjunctivitis ( KC ) , caused by Chlamydia trachomatis infection, is rife in 57 countries in the world at present. The World Health Organization ( WHO) listed the global alliance to eliminate blinding trachoma by 2020 as one of top priorities of its blindness prevention in 1998. A simplified classification system for identifying and naming trachoma, designated by WHO, and the SAFE strategy based on community intervention were extended continuously in the world in 10 years since then. The trachoma prevalence trend has showed a change compared with that in the past. China has launched the blindness prevention action, aimed to eliminate blinding trachoma by 2016. In this paper, we reviews progress in diagnosis, treatment and epidemic of trachoma since the extension of the SAFE strategy.

ABSTRACT

AlM:To investigate the prevalence and related high risk factors of retinal vessels disease of native Tibetan among the aged 40 and above in Maqin county, Qinghai province, China. METHODS:The cluster sampling method was used to investigate the visual acuity and retinal vessel diseases of the native Tibetan among the aged 40 and above in Maqin county. RESULTS:Totally 2 511 individuals were underwent the survey, among them, 29 cases (37 eyes) were of retinal vessel diseases, the prevalence was 1. 15%, 21 cases (23 eyes) were retinal vein obstruction (0. 84%), 5 cases (10 eyes) were diabetic retinopathy ( 0. 20%), 3 cases ( 4 eyes) were retinal vasculitis (0. 12%). The blindness and low vision of retinal vessels disease were 23 eyes (0. 92%). CONCLUSlON:All the hypertension, hyperglycemia, erythrocytosis, high altitude and weight are the high risk factors of retinal vessel diseases which are the main eyes fundus disease could grow blind.

ABSTRACT

AIM:To make a survey on people suffering trachoma in Gansu province, and to provide evidence for developing trachoma control and prevention therapy. METHODS: We chose the zone on the basis of relative information. Provincial Office of Blindness Prevention carried out the survey in 3 counties including Tange Township of Wushan, Xiqu Township of Minqin and Hulinjia Township of Jishishan from October 14, 2013 to November 23, 2013. One hundred and fifty primary school students were selected, including 72 boys and 78 girls aging from 5a to 10a with the average age of 7. 5y. The targeted students received the fast trachoma assessment by the adoption of simplified trachoma classification system which was recommended by the World Health Organization. RESULTS: No case of active trachoma, trachomatous trichiasis and corneal disease were examined among 150 students. CONCLUSION: The rate of trachoma is low in Gansu province. But we still cannot get the conclusion that there is no epidemic of trachoma in Gansu. And we need to further expand the survey scope to correctly assess the trachoma case and to provide reliable evidence for trachoma prevention and control.

ABSTRACT

High altitude retinopathy ( HAR ) refers to the body which can't adapt to the hypobaric hypoxia environment at high altitude leading to retinal diseases, which typically manifested as retinal hemorrhages, optic disc edema and cotton wool spots. With the development of high altitude medicine, HAR become a hot topic of eye research in recent years. New researches show a significantly higher incidence of HAR, and HAR has a close contact with acute mountain sickness, high altitude cerebral edema and high altitude pulmonary edema. A further study in pathogenesis and prevention measures of HAR will promote the prevention of altitude sickness. Traditional Chinese Medicine has achieved good effects in the prevention of altitude sickness, but the effect and mechanism of herbs on HAR has not been reported. Through read and summarize the relevant literatures and reports, the author will give an overview of the research advances on HAR's pathogenesis and application of Traditional Chinese Medicine.

ABSTRACT

Background Nd: YAG laser posterior capsulotomy is an important way for after cataract.Usually the patient will use glucocorticoid eye drops to treat the anterior chamber inflammation after operation,but there is potential risk of elevating intraocular pressure (IOP).Objective This study was to compare the clinical effectiveness and safety of loteprednol etabonate ophthalmic suspension,tobramycin+ dexamethasone eye drops and fluorometholone eye drops following Nd: YAG laser posterior capsulotomy.Methods A randomized-controlled clinical trail was performed.One hundrcd and seventy-onc cycs of 127 paticnts who received Nd: YAG laser posterior capsulotomy for after cataract were randomly divided into four groups.Loteprednol etabonate ophthalmic suspension,fluorometholone eye drops,tobramycin+dexamethasone eye drops and systane eye drops was topically administered respectively in the four groups after laser posterior capsulotomy and 6 times per day for 5 days.IOP was measured with Goldmann tomometer 1 hour before operation and 1 hour,1 day,3 days and 7 days after operation.The ocular anterior segment inflammatory response was examined under the slit lamp and scored based on the Peizeng criteria.Written informed consent was obtained from each patient before any relevant medical procedure.Results The IOP was (18.2 ±4.7),(20.1 ±5.7),(18.7±5.5),(19.0 ±4.1),(19.5 ±3.5) mmHg in various time points in the loteprednol etabonate group; (18.7 ±5.3),(20.9±5.7),(21.3±4.5),(21.0±4.9),(22.5±6.5) mmHg in the fluorometholone eye drops group ; (17.9± 6.3),(20.3 ± 6.1),(23.0 ± 3.7),(24.7 ± 4.9),(24.5 ± 6.5) mmHg in the tobramycin +dexamethasone group and(18.4±6.3),(20.7±3.7),(22.7±6.5),(19.6±4.8),(18.5±3.5) mmHg in the systane group,showing a significant difference among the 4 groups (Fgroup =3.876,P =0.023).With the time lapse,the IOP was gradually reduced in the loteprednol etabonate group and systane group,but that in the fluorometholone group and tobramycin+dexamethasone group was elevated,showing a significant difference among them (Ftime =3.801,P =0.031).No any ocular and systemic adverse effect was found in various groups.The percentage of grade 1 and 2 of aqueous inflammatory cells was lower in the loteprednol etabonate group and tobramycin+dexamethasone group than the fluorometholone group and fluorometholone group and systane group(H =8.276,P =0.012).The percentage of Ⅰgrade of aqueous flare was 8% in the loteprednol etabonate group,22% in the fluorometholone group,18% in the tobramycin+dexamethasone group and 30% in the systane group,with a significant difference among them (H=9.305,P=0.000).Conclusions The use of corticosteroid eye drops can relieve the inflammatory response of ocular anterior chamber after Nd: YAG laser posterior capsulotomy.Loteprednol etabonate ophthalmic suspension has a better anti-inflammatory effect and less influence on IOP.

ABSTRACT

Inorganic/polymer hybrid star polylactic acid (POSS-PLA) was obtained through ring-opening polymerization of lactide by using polyhydroxyl cage silsesquioxane (POSS-OH) as the core and tin (II) octoate as the catalyst. The star polylactic acid (POSS-PLA) was used to modify sodium alginate hydrophobically and a drug carrier was obtained. The drug release behavior was investigated by using ibuprofen as the model drug. The results showed that the drug loading rate could be improved and the release rate was postponed with an increase of POSS-PLA content in the carries. The release mechanism gradually changed from the first-order to the zero-order pattern after the modification.
